The Challenge:
Coordinating and delivering patient care are complex challenges health plans, medical groups, employers, and other health care organizations face every day. Eden was offered the opportunity to develop resources that would help these audiences meet these challenges and overcome barriers to improve quality of care in specific areas.

The Eden Solution:
In partnership with the National Commission on Quality Assurance (NCQA), Eden developed a unique publication series providing in-depth examination of the quality-improvement landscape and best
practice studies in the areas of cardiovascular disease, depression, diabetes, and care for older adults.
